Job summary

Crescent Hospice is seeking a Per Diem Hospice Aide (CNA) to provide hands-on end-of-life care, working closely with our nursing team to meet patients' physical, emotional, and personal needs with dignity and compassion.

Qualifications

• High School Diploma or GED is required.
• Current unencumbered CNA certificate in the state.
• Minimum 1 year CNA experience in acute or long-term care; hospice a plus.
• CPR certification if state-mandated.
• Valid driver’s license, vehicle insurance, and dependable transportation.

Responsibilities

• Provide personalized care following the plan of care with dignity.
• Assist with personal care such as bathing, hygiene, and grooming.
• Deliver exceptional customer service and support for patients.
• Support families by sharing knowledge and guidance.
• Maintain accurate documentation in patient records and agency systems.
