Job summary

Altus Hospice seeks a Per Diem Chaplain to provide spiritual and emotional support to patients and families, guiding them through life’s meaningful moments. Join an IDT team dedicated to compassionate end-of-life care with a mission-driven culture.

Responsibilities include spiritual counseling, grief support, and collaboration with care teams to ensure holistic, dignified care. Strong interfaith experience and CPE are valued in this role.

Qualifications

• Graduate from an accredited seminary or school of theology (BD, MDiv, or equivalent) or related pastoral certification.
• Minimum of one unit of Clinical Pastoral Education (CPE) strongly preferred.
• Comfortable working in interfaith settings and able to accept diverse beliefs.
• Experience in hospice, hospital, or community health care settings preferred.
• Must be able to handle stress related to end-of-life care and grief.
• Driver's license and reliable transportation; proficient with EMR systems.

Responsibilities

• Assess spiritual and emotional needs as part of the interdisciplinary care plan.
• Provide spiritual counseling and comfort for patients and families of all faiths.
• Offer grief and bereavement support during and after the hospice journey.
• Collaborate with the Program Director and care team to coordinate spiritual care.
• Communicate effectively with hospice staff and community partners to ensure continuity of care.
• Maintain accurate documentation in EMR per regulatory standards.
