Job summary

Venture Forthe Inc. seeks a compassionate Home Infusion Therapy Registered Nurse (RN) to deliver high-quality infusion services in patients’ homes.

You will provide hands-on care, educate patients, coordinate care, and collaborate with the care team in a mission-driven environment. This role requires a current New York State RN license, 1 year of acute care experience preferred, and infusion therapy expertise including IV, PICC, central lines, biologics, and TPN.

Qualifications

• Current NYS RN license.
• Graduate of an accredited nursing program.
• At least 1 year of acute care nursing experience preferred.
• Infusion therapy experience strongly preferred, including IV therapy, PICC lines, central line care, biologics, TPN, IV antibiotics, and hydration therapy.
• Home care experience preferred but not required.
• Strong communication, organizational, and critical thinking skills.
• Ability to work independently in varied home environments.

Responsibilities

• Provide specialized home infusion therapy services, including IV initiation, monitoring, dressing changes, blood draws, and pump programming.
• Assess patients and develop individualized infusion plans of care.
• Educate patients and caregivers on infusion therapy, medications, line care, and treatment expectations.
• Collaborate with providers, pharmacies, and interdisciplinary care teams to ensure safe and effective care.
• Monitor patients for infusion reactions, side effects, and clinical changes.
• Maintain accurate and timely clinical documentation.
• Support quality improvement initiatives and ongoing competency programs.
• Assist with training and supervision of paraprofessional and nursing staff.

Work Environment

This position involves travel to patient homes and community settings throughout the service area. The role may require lifting and moving equipment and supplies in accordance with job requirements.
