Job summary

Venture Forthe, Inc. is seeking a Home Infusion Therapy Registered Nurse (RN) to deliver high-quality infusion services in patients’ homes. You will perform IV initiation, monitor therapy, educate patients and caregivers, and coordinate with the care team.

This role requires NY RN license and 1 year of acute care experience preferred; infusion experience is highly valued. Travel within the service area is required, with an emphasis on compassionate, independent patient care.

Qualifications

• Current New York State Registered Nurse (RN) license.
• Graduate of an accredited nursing program.
• At least 1 year of acute care nursing experience preferred.
• Infusion therapy experience preferred (IV therapy, PICC lines, central line care, biologics, TPN, IV antibiotics, hydration).
• Home care experience preferred but not required.
• Strong communication and organizational skills.
• Ability to work independently in varied home environments.

Responsibilities

• Provide specialized home infusion therapy services, including IV initiation, monitoring, dressing changes, blood draws, and pump programming.
• Assess patients and develop individualized infusion plans of care.
• Educate patients and caregivers on infusion therapy, medications, line care, and treatment expectations.
• Collaborate with providers, pharmacies, and interdisciplinary care teams to ensure safe and effective care.
• Monitor patients for infusion reactions and clinical changes.
• Maintain accurate and timely clinical documentation.
• Support quality improvement initiatives and ongoing competency programs.
• Assist with training and supervision of paraprofessional and nursing staff.

Work Environment

This position involves travel to patient homes and community settings throughout the service area. The role may require lifting and moving equipment and supplies in accordance with job requirements.
