Job summary

Allcare Home Health is looking for a Per Diem Registered Nurse (RN) in Rosemead, California. This position requires skilled nursing visits to patients in the San Gabriel Valley. Qualifications

• At least one year of nursing experience, including in Home Health or similar environment.
• Ability to work independently and efficiently manage time.
• Knowledge of OASIS documentation is a plus.

Responsibilities

• Perform Start of Care, Resumption of Care, Recertification, and routine skilled nursing visits.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Complete accurate and timely documentation.
• Perform medication reconciliation at each visit.
• Provide patient and family education based on diagnosis.
• Collaborate with Care Coordinators and interdisciplinary team.
• Maintain compliance with Joint Commission and agency standards.
