Job summary

At Home Healthcare, a part of LHC Group, is seeking a Registered Nurse in Home Health to provide and direct nursing care to patients in their homes as prescribed by the physician and in line with laws and agency policies.

You will coordinate the plan of care with the health care team, supervise home health aides, complete assessments, and ensure continuity of patient care across settings. This full-time role offers comprehensive benefits and competitive compensation.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with physician and team.
• Supervises home health aides and licensed nurses; assigns tasks per regs.
• Provides required supervisory visits.
• Documents visits, completes comprehensive assessments, and ensures policy alignment.
• Communicates with physicians, patients, and family to ensure quality care.
