Job summary

Dedicated Home Health is expanding its Medicare Home Health team and seeking experienced Registered Nurses for per-diem visits. You will perform OASIS assessments, Start of Care, and subsequent visits while coordinating with physicians and the interdisciplinary team, providing one-on-one care in patients’ homes.

The role emphasizes flexible scheduling, high-quality documentation, and opportunities to grow visits as patient census expands.

Qualifications

• Current, unrestricted Florida RN license.
• Medicare-certified Home Health experience.
• Hands-on OASIS assessment experience.
• Experience completing SOC, ROC, Recertification and Discharge visits.
• Strong head-to-toe assessment and clinical documentation skills.
• Understanding of Medicare home health requirements and plan-of-care coordination.
• Ability to communicate effectively with physicians and the interdisciplinary care team.
• Reliable transportation and ability to travel within the assigned territory.
• Commitment to timely documentation and high-quality patient care.

Responsibilities

• Provide skilled nursing care in patients' homes.
• Coordinate care with physicians, therapists and interdisciplinary team.
• Complete comprehensive OASIS assessments.
• Perform Start of Care and other skilled visits.
• Medication reconciliation and patient/caregiver education.
• Manage disease processes and chronic conditions.
• Wound assessment and treatment when ordered.
• Monitor changes in patient condition and report findings.
• Update and coordinate the patient’s plan of care with the team.

Medicare Home Health and OASIS experience are strongly preferred.

The Role

As a Per Diem Home Health Registered Nurse, you will provide skilled nursing care to patients in their homes while coordinating care with physicians, therapists and other members of the interdisciplinary team.

Responsibilities may include:

• Completing comprehensive OASIS assessments
• Start of Care and other skilled nursing visits
• Medication reconciliation and patient/caregiver education
• Disease-process and chronic-condition management
• Wound assessment and treatment, when ordered
• Monitoring changes in patient condition
• Communicating significant findings to physicians and the clinical team
• Updating and coordinating the patient’s plan of care
• Completing accurate and timely clinical documentation
