Dedicated Home Health is growing and is seeking an experienced Registered Nurse (RN) to join our Medicare Home Health team on a per-diem basis with OASIS experience.

The ideal candidate holds a Florida RN license, Medicare-certified home health experience and strong clinical documentation skills, and is ready to provide one-on-one skilled nursing in patients' homes, with flexible visits and territory travel.

Qualifications

• Medicare Home Health and OASIS experience required.
• Florida RN license required.
• Strong assessment and documentation skills.
• Ability to travel within assigned territory.
• Per-diem visits with flexible scheduling.

Responsibilities

• Provide skilled nursing care in patients' homes.
• Complete OASIS assessments.
• Coordinate care with physicians and the interdisciplinary team.
• Maintain timely documentation and plan of care updates.
• Manage Start of Care, ROC, Recertification, and Discharge visits.
