Job summary

Dedicated Home Health is seeking an experienced Registered Nurse (RN) with Medicare home health and OASIS experience for per diem visits in the Florida region.

The ideal candidate has psych-specific experience, strong clinical assessment skills, and a track record of completing SOC, ROC, recertification and discharge visits within Medicare home health guidelines. Travel within the territory is required.

Qualifications

• Florida RN license and Medicare home health experience required.
• Must have PSYCH experience and hands-on OASIS assessment experience.
• Experience with Start of Care (SOC), Resumption of Care (ROC), Recertification and Discharge visits.
• Strong head-to-toe assessment and clinical documentation skills.
• Ability to travel within the assigned territory and communicate with the care team.

Responsibilities

• Complete comprehensive OASIS assessments.
• Perform SOC/ROC and other skilled nursing visits.
• Conduct medication reconciliation and patient/caregiver education.
• Manage disease processes and chronic conditions; provide wound care when ordered.
• Update and coordinate the patient’s plan of care with physicians and therapists.

Registered Nurse (RN) – PSYCH Per Diem Medicare Home Health | OASIS Experience

Dedicated Home Health is growing and is seeking experienced Registered Nurses (RNs) to join our Medicare Home Health team on a per-diem basis.

What We're Looking For

Medicare Home Health and OASIS experience are strongly preferred.

Qualified candidates should have:

• Current, unrestricted Florida Registered Nurse (RN) license
• Medicare-certified Home Health experience
• MUST HAVE PSYCH EXPERIENCE
• Hands‑on OASIS assessment experience
• Experience completing Start of Care (SOC), Resumption of Care (ROC), Recertification and Discharge visits
• Strong head-to-toe assessment and clinical documentation skills
• Understanding of Medicare home health requirements and plan-of-care coordination
• Ability to communicate effectively with physicians and the interdisciplinary care team
• Reliable transportation and ability to travel within the assigned territory
• Commitment to timely documentation and high-quality patient care

The Role

As a Per Diem Home Health Registered Nurse, you will provide skilled nursing care to patients in their homes while coordinating care with physicians, therapists and other members of the interdisciplinary team.

Responsibilities may include:

• Completing comprehensive OASIS assessments
• Start of Care and other skilled nursing visits
• Medication reconciliation and patient/caregiver education
• Disease-process and chronic-condition management
• Wound assessment and treatment, when ordered
• Monitoring changes in patient condition
• Communicating significant findings to physicians and the clinical team
• Updating and coordinating the patient’s plan of care
• Completing accurate and timely clinical documentation
