Job summary

Dedicated Home Health - FL is seeking an experienced RN to join our Medicare Home Health team on a per-diem basis. You will perform skilled nursing visits in patients’ homes, coordinating care with physicians and the interdisciplinary team.

The role emphasizes OASIS assessments, Start of Care visits, and accurate documentation to support patient recovery and independence. Florida RN licensure and Medicare Home Health experience are highly valued.

Qualifications

• Current, unrestricted Florida Registered Nurse (RN) license.
• Medicare-certified Home Health experience strongly preferred.
• Hands-on OASIS assessment experience.
• Experience completing Start of Care (SOC), Resumption of Care (ROC), Recertification and Discharge visits.
• Strong head-to-toe assessment and clinical documentation skills.
• Understanding of Medicare home health requirements and plan-of-care coordination.
• Ability to communicate effectively with physicians and the interdisciplinary care team.
• Reliable transportation and ability to travel within the assigned territory.
• Commitment to timely documentation and high-quality patient care.

Responsibilities

• Completing comprehensive OASIS assessments
• Start of Care and other skilled nursing visits
• Medication reconciliation and patient/caregiver education
• Disease-process and chronic-condition management
• Wound assessment and treatment, when ordered
• Monitoring changes in patient condition
• Communicating significant findings to physicians and the clinical team
• Updating and coordinating the patient’s plan of care
• Completing accurate and timely clinical documentation

Dedicated Home Health is growing and is seeking experienced Registered Nurses (RNs) to join our Medicare Home Health team on a per-diem basis.

The Role
