Job summary

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a per diem home health position in Scotland County, NC. Responsible for visiting adult and geriatric clients and implementing rehabilitative care across designated territories with a multi-disciplinary team.

Requirements include North Carolina PT licensure and at least 1 year of PT experience. Strong independence, organization, and EMR proficiency are preferred; 2–3 days per week availability is required.

Qualifications

• Active North Carolina PT license.
• Minimum 1 year PT experience.
• Graduate of an APTA-approved PT program.
• Ability to work independently.
• Strong interpersonal, organizational, and problem solving skills.
• Computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform mobility, strength, balance, and safety assessments.
• Develop and revise individualized care plans with therapeutic exercises.
• Educate clients and families on rehabilitative care for independent living.
• Document observations and interventions in EMR.
