Job summary

Metro Physical & Aquatic Therapy is seeking a motivated, per diem Occupational Therapist to provide in-home care on the North Shore in Nassau County, NY. We welcome graduates and experienced OT applicants who want flexible scheduling and a supportive team.

Responsibilities include 1:1 treatment, patient assessment, developing and implementing therapy plans, and documenting visits. NY licensure or eligibility is required; travel to patients’ homes within the geographic area is expected.

Qualifications

• High-quality communication with patients and care teams.
• Willingness to travel to patients' homes within the assigned geographic territory.
• Licensure as an Occupational Therapist in NY or eligibility is required or preferred.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care.
• Educate family members, home health aides, and other caregivers on proper therapy techniques.
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Must be willing to treat geriatric patients.
