Job summary

Gateway Longview's Intensive Preventive Services seeks a per diem Supportive Caseworker to deliver intensive, strength-based case planning for high-risk families referred by the Erie County Department of Social Services, during evenings and weekends.

The role involves in-home/remote outreach, crisis support, transportation when needed, and close collaboration with a diverse team to promote safety, permanency and wellbeing.

Qualifications

• Bachelor's or Master's degree in a human services related field.
• Experience in human services settings preferred.
• Valid New York State driver’s license and auto insurance.

Responsibilities

• Engage families in-home or by phone for at least 1 hour daily to support case plans.
• Assess safety and well-being; address elevated risk concerns during contact.
• Complete progress notes within 48 hours detailing goals and outcomes.
• Use trauma-informed approaches to improve family safety and well-being.
• Provide crisis support and coordinate with agency staff as needed.
• Occasionally transport children and families.
• Participate in meetings and trainings; perform other duties as assigned.
