Job summary

Preferred Care Health Centers is seeking a licensed practical nurse (LPN) to support patients under physician orders and RN supervision. You will monitor patient conditions, provide bedside care, and offer emotional support to patients and families in times of crisis to maximize comfort.

Responsibilities include collaborating with the RN case manager, processing physician orders to maximize comfort, and reporting significant changes.

Qualifications

• Graduate of an accredited practical nursing program.
• Active LPN license in New Jersey.
• 1–2 years clinical experience, hospice/long-term care/home health preferred.
• Understanding of Hospice philosophy and needs of terminally ill.
• Strong problem solving and critical thinking skills.
• Ability to travel within the state of NJ.

Job Summary

Under physician's orders and RN supervision, the Continuous Care (CC) Licensed Practical Nurse (LPN) is responsible for the observation of patient/family needs and providing supportive care and emotional support to the patient/family unit in times of crisis in order to maximize patient comfort and enhance quality of life.

Responsibilities

• Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.
• In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.

Qualifications

• Graduate of an accredited practical nursing program.
• Active Licensed Practical Nurse (LPN) in the State of New Jersey.
• Minimum of 1-2 years of clinical experience, preferably in hospice, long-term care, home health, or acute care.
• Understanding of Hospice philosophy and needs of terminally ill.
• Strong problem solving and critical thinking skills.
• Ability to travel within the state of NJ
