Qualifikationen

• Current clinical licensure strongly preferred.
• One year recent relevant experience required in an acute care setting.

• Works with physicians and the team to develop a plan of care for assigned patients.
• Monitors patient progress and identifies barriers to care.
• Actively involved in discharge planning and care coordination.
