Job summary

Rehabilitation Hospital of Northern Indiana in Mishawaka, Indiana is seeking a PRN Case Manager to join our inpatient team. This per diem role supports our rehabilitation services, coordinating care for patients recovering from serious injuries or illnesses.

You will work with physicians and a multidisciplinary team to plan care, monitor progress, and assist with discharge planning, ensuring alignment with our mission and guiding principles.

Qualifications

• Current clinical licensure strongly preferred.
• At least 1 year of recent relevant experience in an acute care setting is required.

Responsibilities

• Works with physicians and the multidisciplinary team to develop a plan of care for assigned patients.
• Monitors patient care progress through assessments and records to ensure desired outcomes.
• Identifies and resolves barriers to effective patient care.
• Actively participates in discharge planning.
• Aligns daily practice with the hospital's mission and guiding principles.
