By The Bay Health is seeking an After-Hours Nurse to deliver skilled nursing and hospice care in Marin County with occasional coverage in San Francisco and Sonoma counties. This per diem role requires a California RN license and at least one year of recent nursing experience, ideally in home health or hospice.

The position emphasizes patient- and family-centered care, collaboration with physicians, and adherence to CDPH vaccine requirements.

Qualifications

• Active RN license in California.
• 1 year of professional nursing experience in the past 3 years or a nursing degree.
• Experience in acute care/home health/hospice is a plus.

Responsibilities

• Provide continuing care to patients and families in accordance with policy and regulations.
• Assess, plan, implement, and evaluate nursing services per care plan.
• Coordinate with attending physicians and community resources for optimal patient care.
• Maintain regular communication with team leaders about patient status and issues.
