Job summary

By The Bay Health is a nonprofit organization delivering hospice and home-based care in the San Francisco Bay Area. We seek an After‑Hours Nurse to provide skilled nursing and hospice services to maximize comfort and quality of life for patients and families.

The role requires bilingual Cantonese, an active California RN license, and experience in acute care, home health, or hospice. Schedule includes Tue/Fri 3:30 pm–12:00 am and Sat/Sun 8:30 am–5:00 pm, with competitive pay and benefits.

Qualifications

• Bachelor's or associate degree in nursing required, CA RN license.
• Experience in acute care, home health, or hospice preferred.

Responsibilities

• Provide continuing nursing care to patients and families per plan of care.
• Under supervision of After-Hours Nurse, assess, plan, implement and evaluate nursing services.
• Coordinate with physicians and community resources for optimal patient care.
• Maintain communication with Team Leaders regarding patient status.
