• Establish and implement plan of care with patient goals; monitor outcomes and adjust treatment.
• Complete discharge planning with home programs and family instruction; coordinate with care team.
• Document patient care per policy; maintain productivity standards.
• Ensure patient safety and infection control measures.
• Assist with supervision of therapy staff and students when applicable.
