Job summary

Advocate Health is seeking a licensed Physical Therapist for part-time rehabilitation duties at Advocate Condell Medical Center. Expect evaluations, treatment planning, and discharge planning with patient education and collaboration with the care team.

Position requires Doctorate in Physical Therapy, Illinois state licensure, and completion of required certifications. Partial weekend and holiday coverage may be required in a team-based setting with comprehensive benefits.

Responsibilities

• Interpret referrals and perform musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establish and implement plan of care; monitor outcomes and adjust treatment; communicate with physicians and teammates.
• Complete discharge planning including home programs and patient/family education.
• Document patient care and progress following department policy; manage scheduling and billing/documentation.
• Maintain safety, infection control measures and assist in supervision as requested.
