Job summary

Southeastern Integrated Care LLC seeks a Peer Support Specialist in Shallotte, NC. You will provide coaching, mentoring, and consultation to adults with MH/SA disabilities, promoting recovery, self-advocacy, and independence through peer-led supports and community integration.

As part of an interdisciplinary team, you’ll deliver wellness self-management coaching, assist with crisis planning, and help clients access housing, education, and employment resources while maintaining HIPAA-compliant

Qualifications

• Collaborate with enrolled members at least 5 days per week.
• Document direct service delivery per Medicaid and CSEUC standards.
• Read medical records to locate pertinent information quickly.
• Write complete service notes.
• Able to present information clearly to consumers/public.

Responsibilities

• Provide wellness self-management and recovery-focused peer supports.
• Assist clients with decision-making and self-determination.
• Teach functional skills related to meds, finances, health care, and daily living.
• Model recovery values and empower clients to exercise their rights.
• Develop, modify, and implement a Wellness Recovery Action Plan.
• Link clients to community resources and provide case management.
• Maintain confidentiality per HIPAA and CFR-42C.
