Qualifications

Responsibilities

• Carry out team duties under the Lead Peer Mentor.
• Provide transportation and on-site support.
• Mentor, problem-solve, and encourage clients.
• Communicate with clients to answer questions and strengthen relationships.
• Provide practical help and advocacy to obtain daily living needs.
• Coordinate social activities and community engagement.
• Document all activities in HIPAA-compliant manner.
