Job summary

Carolina Outreach in Wilmington, NC is seeking a full-time Community Support Team (CST) Professional Team Lead to provide community-based therapy and case management for adults with serious mental health challenges.

You will conduct initial assessments, provide individual and family therapy, develop person-centered plans, monitor symptoms, and coordinate referrals while maintaining timely clinical documentation.

Qualifications

• Master’s degree from an accredited university in Counseling or a related field.
• Valid licensure to provide mental/behavioral healthcare in NC (LCSW, LCSWA, LCMHC, LCMHCA, LMFT, or Licensed Psychologist).
• A minimum of two years of experience working with individuals with mental and/or behavioral health issues that include writing person-centered plans, conducting effective group therapy and providing mental health diagnoses.
• General working knowledge of the human services delivery system in North Carolina.
• Valid driver’s license, auto insurance, and reliable transportation.
• Employee supervisory experience preferred.
• Experience with Electronic Medical Records (EMR) preferred.

Responsibilities

• Provide individual, family, and/or group therapy sessions.
• Maintain accurate and timely clinical documentation.
• Collaborate with care teams as needed.
• Maintain compliance with all clinical and regulatory standards.
• Participate in supervision (if applicable based on licensure level).
