Job summary

Baltic Street Wellness Solutions in New York is seeking a Peer Support Specialist to help adults moving from adult homes back into the community. You will use your lived experience to build trust, provide one-on-one and group support, and connect clients with local services.

This role requires NYS Certified Peer Specialist or the ability to become certified within 6 months, strong advocacy and communication skills, and the ability to travel to different homes in the boroughs.

Qualifications

• Minimum one year of peer support experience.
• New York State Certified Peer Specialist or must certify within 6 months of hire.
• Ability to model hope and wellness for people with mental illness.
• Experience navigating NYS mental health and substance use systems.
• Basic computer skills and reliability with documentation.

Responsibilities

• Offer peer relationships that foster trust, hope, and wellness.
• Be a positive role model reflecting recovery and personal wellness.
• Provide one-on-one and group support post-relocation.
• Teach community adjustment and wellness self-management.
• Assist with WRAP and personal wellness planning.
• Identify and reconnect with family and natural supports.
• Provide education and resources to support self-determination and advocacy.
• Maintain timely, accurate documentation of activities.
• Attend trainings and meetings as required.
• Travel to different homes within boroughs; afternoon, evening, and weekend availability.
