Job summary

LifePath Systems in Plano, TX is seeking a dedicated Family Partner to support families of youth with emotional or behavioral health challenges. You will use your lived experience to empower caregivers, collaborate with a treatment team, and help families navigate services.

This community-based role requires travel within Collin County and may involve occasional trips for credentialing. Strong communication, reliability, and ability to work flexible hours are essential.

Qualifications

• Must have either a High School Diploma or GED and at least 2 years’ experience navigating a child-serving system.
• Or Bachelor’s degree in a human service related field that meets criteria for Qualified Mental Health Professional AND at least 1 year experience navigating a child-serving system.

Responsibilities

• Introduce the family to the treatment process; model self-advocacy skills; provide information; make referrals.
• Provide treatment interventions based on evidenced-based practices, certifications, and scope; assist in identifying natural/non-traditional and community supports.
• Work in a trauma-informed, person/family centered model and travel within Collin County as needed.
