Blue Ridge Health is currently seeking a Peer Extender to be part of our Tailored Care Management team.
The Peer Extender plays a unique and vital role in supporting one’s journey of recovery. They serve on an interdisciplinary tailored care management team whose focus is to deliver integrated, whole person care addressing a member’s physical health, behavioral health, long‑term services and health needs and/or pharmacy needs. Responsibilities include:
This role requires flexibility to transport clients/patients to organization-sponsored services or resources that address their basic and clinical needs. Drivers transporting clients/patients or driving company vehicles are to be age 21 or greater. For those using their personal vehicles to transport clients/patients, a certificate of insurance is required demonstrating at least the minimum required NC auto liability insurance.
