Job summary

Blue Ridge Health in Hendersonville, NC seeks a part-time Patient Navigator to support access to care and coordinate services within the clinic team. Responsibilities include screening for eligibility, assisting with applications for fee programs and insurance, and referrals to partner agencies.

CAC and SHIIP certifications are required within one month of hire. Strong knowledge of social determinants of health and nonprofit healthcare settings is preferred.

Qualifications

• Bachelor’s degree in healthcare or social services preferred; equivalent combination of education and experience acceptable.
• Familiarity with Medicaid, Medicare, Health Insurance Marketplace and other social safety net programs preferred.
• Ability to obtain CAC and SHIIP counselor certifications within one month of hire.

Responsibilities

• Screen patients for eligibility and assist with applications for sliding scale, subsidized insurance, charity care, medication assistance, housing/food/transportation issues.
• Conduct enrollments and refer to partner agencies as appropriate.
• Provide CAC and SHIIP counseling and educate patients about advocacy and voter registration.

What You'll Do:

The Patient Navigator is a member of the clinic interdisciplinary team responsible for facilitating improved health care access and quality for BRH patients through advocacy and care coordination to address social determinants of health (SDH). Responsibilities include:

• Screens patients for the eligibility of and assists with completion of needed applications and paperwork for:
  • sliding fee scale/discount fee program
  • subsidized health insurance programs
  • charity care programs, including local hospital applications
  • medication assistance programs
  • addressing legal issues
  • other programs that assist with social determinants of health (i.e. food insecurity, housing insecurity, transportation insecurity)
• Conducts enrollments into programs when possible, and provides referrals to partner agencies, as appropriate.
• Maintains certification as a Certified Application Counselor for the organization and completes all required training(s) to maintain certification.
• Provides Senior’s Health Insurance Information Program (SHIIP) counseling to Medicare beneficiaries, as requested.
• Educates patients on health center advocacy and voter registration and provides related assistance.

What We're Looking For:

• Bachelor’s degree in healthcare or social services related field, preferred. Equivalent combination of education and experience is acceptable.
• Familiarity with Medicaid, Medicare, Health Insurance Marketplace, private health insurance, SNAP, and other local, state, or national social safety net programs strongly preferred.
• Ability to be certified as a Certified Application Counselor (CAC) and Senior’s Health Insurance Information Program (SHIIP) counselor within one month of hire.
• Valid driver’s license and reliable transportation, required
