Job summary

Mercy Clinic Fenton is seeking a dedicated RN Nurse Navigator to support the Mercy Kids Weight & Wellness Program. This role links patients, families, and providers to streamline weight management, education, and ongoing care.

You will help patients navigate referrals, follow-ups, and community resources with compassionate, patient-centered care. The RN will work within a pediatric specialty team, coordinating care from referral through follow-up while promoting adherence and positive patient

Qualifications

• Graduate of an accredited school of nursing.
• Current Registered Nurse (RN) license in the applicable state.
• CPR Certification or ability to obtain within 90 days of employment.
• Strong clinical assessment and patient care skills.
• Ability to coordinate care and educate patients and families.

Responsibilities

• Escort patients to examination rooms and prepare them for provider visits.
• Coordinate appointments, referrals, testing, and follow-up services.
• Provide education regarding treatment plans, wellness goals, and available resources.
• Facilitate communication between patients, families, providers, and specialists.
• Help identify and address barriers to care and treatment adherence.
• Support patient engagement and continuity of care.
