Qualifications

• At least 2 years of health care experience, preferably in care management or related setting.
• Licensure/certification required only if dictated by state contract or regulation.
• Valid driver’s license, reliable transportation, and adequate auto insurance for travel.
• Knowledge of community resources and ability to navigate services.
• Ability to work independently with minimal supervision and be self‑motivated.
• Strong verbal and written communication skills; detail‑oriented and organized.

Responsabilités

• Completes member assessments per timelines to determine care coordination eligibility.
• Develops and implements member care plans with input from member, family, and providers.
• Performs telephonic, in‑person, or home visits as needed.
• Monitors care plans, documents interventions, and suggests changes as needed.
• Maintains ongoing caseload with regular outreach and management.
• Promotes integration of behavioral health, LTSS, and community resources to support continuity of care.
• Facilitates interdisciplinary team meetings and collaboration.
• Uses motivational interviewing and Molina guides to support member change.
• Assesses barriers to care and provides assistance to address concerns.
• Collaborates with licensed care managers and leadership as required.
• Travel 25–40% depending on state and contract requirements.
