HOPE Clinic seeks a Pediatric Weight Management Coordinator to implement, coordinate, and maintain the AAP Healthy Weight Clinic (HWC) program. You will oversee care for 12 pediatric patients and their families across onsite and offsite appointments, education sessions, and care needs.

The role involves organizing sessions, coordinating staff and partners, and conducting program evaluation reports. A strong background in public health data and patient privacy is essential.

Qualificações

• Highschool diploma or GED required.
• Bachelor's degree or current enrollment in healthcare administration, public health, social work, or related field is required.
• Masters degree is preferred.
• 1–2 years healthcare administration, grant writing, or project coordination experience is required.

Responsabilidades

• Learn and master the content of the HWC Program.
• Coordinate implementation logistics and support under supervision.
• Organize sessions onsite at HOPE Clinic – Alief and with community partners.
• Coordinate staff and community resources for the HWC program.
• Provide clinical telephone check-ins with patients twice a month.
• Participate in the HWC virtual learning community.
• Maintain connections with community partners.
• Complete program evaluation reports as required.
• Attend events, training, or activities as needed.
• Support HOPE Clinic’s Mission, Vision and Values.
