Qualifications

• Current RN license for the state where you practice.
• Current Health Certificate per RN contract or state regulation.
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year RN experience preferred.

Responsibilities

• Uses the nursing process to plan, implement, and evaluate patient care for home-visited clients.
• Initiates and revises the plan of care with prescriber as patient needs change.
• Delivers therapeutic, preventive, and rehabilitative nursing actions per diagnosis and goals.
• Intervenes for changes in patient status and reports to physician or supervisor.
• Administers medications and treatments, monitors effectiveness and documents results.
• Reviews plan of care and coordinates education, care, and discharge planning.
