Job summary

The TRC Case Manager role at UC Davis Health assesses community resources for crime victims and families, helps form patient-centered care plans, and guides navigation of health systems and social services, with a strong emphasis on trauma-informed practice.

This on-site, full-time position focuses on patient navigation, collaboration across departments, and trauma recovery program development within the CAARE Center and UCD Trauma Recovery Center.

Qualifications

• Bachelor's degree in related area and / or equivalent experience / training.
• Experience working on a multidisciplinary team supporting mental health therapists.
• Experience working with youth and young adults affected by violence or involved with the Justice System.
• Knowledge of intervention techniques and trauma-informed practices.
