Job summary

Lightways Hospice and Serious Illness Care is seeking a Pediatric Team Leader responsible for overseeing the Pediatric team and directing hospice care. The ideal candidate will have a minimum of 3 years of hospice experience as an RN and prior supervisory experience.

Responsibilities

• Oversee the Pediatric team and direct hospice care.

• Perform quarterly performance check-ins.
• Participate in ride-a-long visits as needed.
• Maintain a small patient caseload.
• Conduct patient and family assessments.

Pediatric Team Leader

The Pediatric Team Leader oversees the Pediatric team and plans, organizes and directs hospice care, and is experienced in nursing, with emphasis on community health education/experience.

Key Responsibilities

• Enhance communication to ensure existing team members receive support in the field.
• Conduct weekly IDT meetings for the Pediatric team.
• Guide new employees with orientation and education in hospice care.
• Perform timely performance check-ins with their team on a quarterly basis.
• Participate in ride‑a‑long visits with their team on a quarterly basis and as needed.
• Maintain a small caseload.
• Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.

Education / Experience

• RN with minimum of 3 years of Hospice experience.
• Prior supervisory experience.
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements; and is in good working order.
