Job summary

Joliet Area Community Hospice is seeking a Pediatric Team Leader to oversee the Pediatric team and coordinate hospice care. This role involves enhancing team communication, conducting meetings, and ensuring quality patient assessments.

The ideal candidate will be a Registered Nurse with at least 3 years of hospice experience, and prior supervisory experience is preferred. Benefits include 401(k) matching and health insurance.

Qualifications

• Registered Nurse (RN) with at least 3 years of hospice experience.
• Prior experience in a supervisory role is essential.
• Valid driver's license and a fully insured vehicle required.

Responsibilities

• Oversee the Pediatric team and direct hospice care.
• Conduct weekly IDT meetings for team collaboration.
• Guide new employees in hospice care orientation.
• Perform quarterly performance check-ins with team members.
• Participate in ride-along visits with team members.
• Maintain a small caseload of patients.
• Conduct comprehensive assessments of patients and families.

Pediatric Team Leader

The Pediatric Team Leader oversees the Pediatric team and plans, organizes and directs hospice care, and is experienced in nursing, with emphasis on community health education/experience.

Key Responsibilities

• Enhance communication to ensure existing team members receive support in the field.
• Conduct weekly IDT meetings for the Pediatric team.
• Guide new employees with orientation and education in hospice care.
• Perform timely performance check‑ins with their team on a quarterly basis.
• Participate in ride‑along visits with their team on a quarterly basis and as needed.
• Maintain a small caseload.
• Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.

Education/Experience

• RN with minimum of 3 years of Hospice experience.
• Prior supervisory experience.
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements; and is in good working order.
