Job summary

Responsibilities

• Provide patient care within licensed scope and contribute to treatment goals.
• Establish and adjust written treatment plans with patient/family input.
• Collaborate with the interdisciplinary team and assist with discharge planning.
• Document patient status, progress, and discharge summaries per standards.
• Supervise CFY and assist with staff instruction as needed.
• Utilize relevant software/hardware to support clinical responsibilities.
