Job summary

Amazing Care Home Health Services in Utah County is seeking a Speech-Language Pathologist to deliver home health speech therapy services to clients of all ages. The role focuses on evaluating, planning, and implementing treatment plans in home and community settings, coordinating with physicians and caregivers.

Qualifications

• A master's or doctoral degree in speech-language pathology (SLP).
• ASHA Certification or PRAXIS exam with Clinical Fellow/Externship.
• Speech-Language Pathologist (SLP) license in Utah.
• Active BLS certification.
• Valid driver's license and auto insurance.
• A minimum of 1-year clinical experience in Speech Language Pathology is preferred, but not required.

Responsibilities

• Performs evaluations and interprets assessment results, develops a plan of care with skilled interventions and goals.
• Assists clients to develop or regain cognitive, speech, language, feeding/swallowing, and social/emotional functioning.
• Coordinates care with referring physician and other health care professionals to ensure safe, appropriate care.
• Instructs client and family/caregiver in the treatment plan as indicated
• Identifies needs for other services and makes referrals as appropriate
• Prepares and submits documentation per organization policy
• Maintains clinical competency in speech-language pathology

Education

Master's or Doctoral degree in Speech-Language Pathology (SLP)

ASHA Certification or PRAXIS with CF/Externship

Utah SLP License

Active BLS certification

Valid driver's license & auto insurance

1+ year clinical experience in Speech Language Pathology (preferred)

We are seeking a Speech Language Pathologist to join our team in the Utah County area. A speech-language pathologist (SLP) will implement treatment plans to assist clients of all ages with cognitive, speech, language, feeding/swallowing, and/or social/emotional functioning by developing and administering speech therapy services in the home and community settings.

• Provides compassionate and high-quality care that meets the needs of the client and family by performing evaluations and interpreting assessment results, developing a plan of care including skilled interventions and goals appropriate to the client's needs, and creating and implementing speech and language treatment plans in conjunction with the physician
• Assists pediatric and adult patients to develop or regain cognitive, speech, language, feeding/swallowing, and/or social/emotional functioning and improves their level of independence and quality of life
• Coordinates care with referring physician and other health care organizations and personnel to ensure safe, appropriate, and effective care for the client
• Instructs client, family/caregiver, and other organization health care personnel in the client's treatment plan as indicated
• Identifies client and family/caregiver needs for other services and refers as appropriate
• Prepares and submits documentation per organization policy
• Maintains clinical competency in the theory and practice of speech-language pathology

Mileage provided for commuting between patient visits

Schedule

Part-Time Days ,10-20 visits per week

Requirements

• A master's or doctoral degree in speech-language pathology (SLP)
• ASHA Certification or documentation of passing PRAXIS exam and completion of a Clinical Fellow/Externship
• Speech-Language Pathologist (SLP) License in the state of Utah
• Active BLS certification
• Valid driver's license and auto insurance
• A minimum of 1-year clinical experience in Speech Language Pathology is preferred, but not required
