Job summary

Amazing Care Home Health Services in Logan seeks a Speech Language Pathologist to join our team in the home and community settings. The role develops and administers speech therapy services to clients of all ages, focusing on cognitive, speech, language, feeding/swallowing, and social/emotional functioning.

The SLP will perform evaluations, create and implement treatment plans, coordinate with physicians, and educate clients and caregivers.

Qualifications

  • Master’s or doctoral degree in speech-language pathology (SLP).
  • ASHA certification or PRAXIS + Clinical Fellow/Externship completed.
  • Utah SLP license.
  • Active BLS certification.
  • Valid driver’s license and auto insurance.
  • 1+ year clinical experience preferred.

Responsibilities

  • Perform evaluations and interpret assessment results; develop care plans with goals.
  • Develop and implement speech and language treatment plans in home/community settings.
  • Coordinate care with referring physician and care teams.
  • Instruct clients and caregivers on treatment plans.
  • Identify needs for additional services and make referrals as needed.
  • Submit documentation per organization policy.
  • Maintain clinical competency in speech-language pathology.

Job description

We are seeking a Speech Language Pathologist to join our team in the Logan area. A speech‑language pathologist (SLP) will implement treatment plans to assist clients of all ages with cognitive, speech, language, feeding/swallowing, and/or social/emotional functioning by developing and administering speech therapy services in the home and community settings.

  • Provides compassionate and high‑quality care that meets the needs of the client and family by performing evaluations and interpreting assessment results, developing a plan of care including skilled interventions and goals appropriate to the client’s needs, and creating and implementing speech and language treatment plans in conjunction with the physician.
  • Assists pediatric and adult patients to develop or regain cognitive, speech, language, feeding/swallowing, and/or social/emotional functioning and improves their level of independence and quality of life.
  • Coordinates care with referring physician and other health care organizations and personnel to ensure safe, appropriate, and effective care for the client.
  • Instructs client, family/caregiver, and other organization health care personnel in the client’s treatment plan as indicated.
  • Identifies client and family/caregiver needs for other services and refers as appropriate.
  • Prepares and submits documentation per organization policy.
  • Maintains clinical competency in the theory and practice of speech‑language pathology.

Mileage provided for commuting between patient visits.

Schedule

Part‑Time Days, 10–20 visits per week.

Requirements
  • A master’s or doctoral degree in speech‑language pathology (SLP).
  • ASHA Certification or documentation of passing PRAXIS exam and completion of a Clinical Fellow/Externship.
  • Speech‑Language Pathologist (SLP) License in the state of Utah.
  • Active BLS certification.
  • Valid driver’s license and auto insurance.
  • Minimum of 1‑year clinical experience in Speech‑Language Pathology preferred, but not required.
