Job summary

Amazing Care is seeking a Speech Language Pathologist to provide in-home and community-based therapy in Utah County. You will assess, plan, and implement treatment for clients across ages, focusing on cognitive, speech, language, feeding/swallowing, and social/emotional goals, in collaboration with physicians and care teams.

The role offers part-time scheduling (10-20 visits per week) with mileage reimbursement, flexible days, and ongoing professional support within a mission-driven home health

Qualifications

• Master's or doctoral degree in Speech-Language Pathology (SLP).
• ASHA Certification or documentation of passing PRAXIS exam and completion of a Clinical Fellow/Externship.
• Speech-Language Pathologist (SLP) License in the state of Utah.
• Active BLS certification.
• Valid driver's license and auto insurance.
• A minimum of 1-year clinical experience in Speech Language Pathology is preferred, but not required.

Responsibilities

• Implements treatment plans to assist clients of all ages with cognitive, speech, language, feeding/swallowing, and/or social/emotional functioning.
• Develops and administers speech therapy services in home and community settings.
• Coordinates care with referring physicians and health care teams for safe, appropriate care.
• Instructs client and family in treatment plans and goals.
• Identifies needs for other services and referrals as appropriate.
• Prepares and submits documentation per policy.
• Maintains clinical competency in speech-language pathology.

We are seeking a Speech Language Pathologist to join our team in the Utah County area. A speech-language pathologist (SLP) will implement treatment plans to assist clients of all ages with cognitive, speech, language, feeding/swallowing, and/or social/emotional functioning by developing and administering speech therapy services in the home and community settings.

• Provides compassionate and high-quality care that meets the needs of the client and family by performing evaluations and interpreting assessment results, developing a plan of care including skilled interventions and goals appropriate to the client's needs, and creating and implementing speech and language treatment plans in conjunction with the physician.
• Assists pediatric and adult patients to develop or regain cognitive, speech, language, feeding/swallowing, and/or social/emotional functioning and improves their level of independence and quality of life.
• Coordinates care with referring physician and other health care organizations and personnel to ensure safe, appropriate, and effective care for the client.
• Instructs client, family/caregiver, and other organization health care personnel in the client's treatment plan as indicated.
• Identifies client and family/caregiver needs for other services and refers as appropriate.
• Prepares and submits documentation per organization policy.
• Maintains clinical competency in the theory and practice of speech-language pathology.

Schedule:
