Job summary

Qualifications

• Master's Degree in Social Work required.
• Unlicensed Social Worker grace period for LCSW eligibility in Massachusetts.
• Social Worker I: 0–1 year clinical experience in medical setting.
• Social Worker II: 2–3 years clinical experience; LICSW required.

Responsibilities

• Provide psychosocial assessments of patients and families with social, emotional, interpersonal and environmental issues.
• Formulate biopsychosocial assessments and treatment plans.
• Collaborate with other professionals to evaluate medical condition and assess client needs as part of the interdisciplinary team.
• Monitor, evaluate, and document client progress according to treatment goals in the care plan.
