Qualifications

• Master's Degree in Social Work required.
• Unlicensed Social Worker: Grace Period for License Eligibility (LCSW - Massachusetts).
• Social Worker I: 0–1 year clinical experience in medical setting.
• Social Worker II: 2–3 years clinical experience in medical setting; LICSW required.

Responsibilities

• Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
• Formulates biopsychosocial assessment, disposition, and treatment plans.
• Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
• Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
