Job summary

Shriners Children's Chicago is seeking a dedicated Social Worker to assess, plan, intervene, and follow up with patients and families across the continuum of care. You will help ensure family-centered care and positive outcomes by coordinating resources with the care team in a pediatric setting.

Qualifications include a Master in Social Work, an Illinois Social Work license, and a Case Management Certification. Bilingual English/Spanish is preferred.

Qualifications

• Three years of social work experience.
• Master in Social Work and Illinois licensure required.
• Case Management Certification preferred or obtainable within 12 months.

Responsibilities

• Documents assessments, interventions, plans and evaluations in accordance with regulatory agency requirements and SHC policy.
• Coordinates the plan of care to achieve desired outcomes and integrates the work of the health care team by coordinating resources and services necessary to accomplish agreed-upon goals.
• Advocates by supporting or recommending services for the protection of the patient's health, safety and rights.
• Collaborates by working with patients, families, caregivers and the health care team locally and within the system to jointly communicate, problem solve, and share accountability for optimal outcomes.
• Arranges services among community agencies, physicians, patients, family, caregivers and others involved in the plan of care.
