In this role you will be responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care. You'll help to provide continuity of care for positive outcomes in our family centered environment.Key Responsibilities:Documents assessments, interventions, plans and evaluations in accordance with regulatory agency requirements and SHC policy.Coordinates the plan of care plan to achieve desired outcomes and integrates the work of the health care team by coordinating resources and services necessary to accomplish agreed-upon goals.Advocates by supporting or recommending services for the protection of the patient's health, safety and rights.Collaborates by working with patients, families, caregivers and the health care team locally and within the system to jointly communicate, problem solve, and share accountability for optimal outcomes.Arranges services among community agencies, physicians, patients, family, caregivers and others involved in the plan of care.Required Qualifications:Masters in Social WorkIllinois Social Work licenseCase Management Certification (CMC, ACM or ANCC) required or the ability to attain certification within 12 months of hire or within 12 months of eligibilityThree years of social work experiencePreferred Qualifications:5-7 years of social work experiencePrevious social worker experience in a pediatric settingBilingual in English/SpanishThe pay range for this position is $69,617.60 - $104,457.60.Compensation is determined based on years of relevant experience and departmental equity.
