HSMC in Gardner, MA seeks a qualified registered nurse to provide infant-toddler developmental support through home-based and center-based services. You will work with a trans-disciplinary team to coach families and coordinate care across providers in the catchment area.

The role requires MA RN licensure and pediatric experience, with flexibility for full-time, part-time, or fee-for-service positions. A sign-on bonus is offered and benefits include health insurance and retirement plans.

Formación

• Bachelor’s degree in nursing or equivalent
• Current RN licensure by MA Board of Registration in Nursing
• Two years of pediatric experience preferred
• Effective verbal and written communication skills
• Proficient with Word, Excel, Adobe, and Office365 including Outlook, OneDrive, and Teams
• Ability to use computer-based technology such as a laptop and cell phone
• Valid driver’s license and reliable transportation
• Bilingual/Bicultural candidates encouraged to apply

Responsabilidades

• Demonstrate knowledge of infant-toddler development.
• Utilize a parent coaching model with families.
• Participate in screenings, assessments, child groups, parent groups, center visits, and home visits as part of a trans-disciplinary team.
• Provide home-based services to families across the catchment area.
• Collaborate with community providers to ensure continuity of services for families including child care, medical, social service, and specialty providers serving children with autism or hearing/vision loss.
• Provide service coordination to assigned caseload including development, monitoring, and review of the Individual Family Service Plan.]
