Job summary

Children’s Healthcare of Atlanta in Atlanta, GA is seeking a registered nurse to perform nursing process and provide direct patient care in a safe, evidence-based manner as part of an interdisciplinary team.

Responsibilities

• Cultivates and maintains effective interaction with medical staff to drive care coordination.
• Communicates with multidisciplinary team to facilitate care coordination.
• Coordinates with home health, payors, and community resources as needed.
• Provides ongoing communication with patient/family and interdisciplinary staff.
• Performs outpatient and clinic care coordination and telehealth support.
• Identifies and resolves potential barriers to patient care.
