OSF HealthCare is seeking an RN Complex Care Manager to lead complex care and chronic disease management for assigned populations. You will assess, plan, implement, and monitor interventions, coordinating with patients, families, and providers to create and follow a comprehensive plan of care.
You will work with the care management leadership to ensure alignment with OSF strategic goals, while providing high-quality care across the continuum and leveraging community resources.

Pay range for this position is $34.25 - $51.13/hour. Actual pay is based on years of licensure. This is a Salaried position.
Work from home 1 day per week after successful completion of 90 day orientation
This position also covers the Central Western Region pediatric population in Galesburg, Monmouth, Kewanne etc with potential need for travel at times for patient needs
POSITION SUMMARY:
The RN Complex Care Manager engages designated patient populations to provide complex care management and chronic disease management and achieve optimal outcomes by assessing, planning, implementing, coordinating, and monitoring interventions. Develops a comprehensive plan of care with the patients, patient designee, providers, and the entire care team to set and monitor goals. Responsible for evaluation and coordinating options and services across the continuum of care. Health care services are coordinated in collaboration with the patient to address the complex medical and clinical needs associated with illness, injury, or chronic disease. Demonstrates the knowledge and skills necessary to provide services to the assigned patient population. Works with the Care Management Division leadership team to ensure alignment with the strategic goals to meet OSF Key Results.
