Job summary

OSF HealthCare is seeking an RN Complex Care Manager to lead complex care and chronic disease management for assigned populations. You will assess, plan, implement, and monitor interventions, coordinating with patients, families, and providers to create and follow a comprehensive plan of care.

You will work with the care management leadership to ensure alignment with OSF strategic goals, while providing high-quality care across the continuum and leveraging community resources.

Qualifications

• Bachelor's Degree in Nursing within 3 years of hire.
• Three years of caring for patients.
• Care management of adult and/or pediatric patients with chronic, complex illnesses.
• Broad knowledge of medical and community resources.
• Effective communication with patients/families from diverse backgrounds.
• Current state RN license or license eligible at time of hire.
