Responsibilities

• Assess patients and coordinate admissions for Colorado Springs and Pueblo.
• Develop, implement and update individualized care plans with physicians.
• Provide skilled nursing care per physician orders and plan of care.
• Document visits and orders remotely and during field visits.
• Coordinate services with therapists, social workers and other providers.
• Supervise LPNs and CNAs; conduct supervisory visits.
• Educate families and caregivers on care plans and safety.
