About Kids Home Health Care is seeking an RN Case Manager to provide skilled nursing care and coordinate pediatric home health services. This hybrid role combines work from home with in-person patient visits across Colorado Springs and Pueblo.

You will assess patients, develop care plans, supervise LPNs/CNAs, and coordinate services with physicians, therapists, and families. Flexible hours tailor around patient visits and administrative duties.

Qualifications

• Active RN license authorizing practice in Colorado.
• Experience coordinating home health services for pediatric patients is shown in duties.

Responsibilities

• Assess patients and coordinate admissions in Colorado Springs and Pueblo.
• Develop and manage individualized plans of care with physicians.
• Provide skilled nursing services per patient plan.
• Monitor progress and communicate with physicians and supervisor.
• Complete remote clinical documentation and OASIS when applicable.
• Coordinate services across the care team and document care coordination every 30 days.
• Arrange additional services and referrals with physician and supervisor.
• Supervise LPNs and CNAs; document supervisory visits.
• Educate caregivers and clinical staff to promote safe, consistent care.
• Plan transitions and discharge with families and care team.
• Maintain clear communication and schedule changes; participate in on-call coverage and holiday visits.
