AuthoraCare Collective seeks a PRN RN Home Care Case Manager to join the Kids Path program serving Burlington, Chapel Hill and Durham. The role focuses on pediatric home health, coordinating care and collaborating with an interdisciplinary team in patients' homes.

The position requires NC RN licensure, pediatric/home health experience, and CPR/BLS certification. Flexibility for travel and variable hours is expected, with training available for pediatrics.

Formación

• Graduate of an accredited School of Nursing.
• Must be currently licensed as an RN in North Carolina and in good standing with NCBON.
• At least one year of experience in an acute or home setting; pediatric experience preferred.
• Bachelor's degree in nursing preferred.
• Familiarity with Medicaid, and/or home health a plus.

Responsabilidades

• Manage RN case load for Kids Path program in collaboration with the interdisciplinary team.
• Assess and identify patient/caregiver complications and establish care goals.
• Work with physician to establish and implement medical plan of care.
• Establish and maintain therapeutic relationships with patients and caregivers; communicate effectively with all stakeholders.
• Occasional after-hours availability and travel to patient homes within the service area.
