Job summary

ACG Hospice is seeking a Pediatric Registered Nurse to join Hands of Hope, our pediatric program. You will evaluate pediatric patients, create care plans, and communicate with patients, families, and the care team to deliver compassionate, quality care in a home-based hospice setting.

You will be part of a supportive leadership team with professional development opportunities and room for advancement within our hospice network.

Qualifications

• Heart to serve patients and families with the best possible care.
• RN license in good standing and educational requirements met.
• 3 years of clinical/home health nursing experience and 1 year pediatric experience.
• Reliable transportation and ability to lift 80-100 lbs as required.

Responsibilities

• Evaluate pediatric patients and develop care plans.
• Communicate with patient, family and care team to coordinate care.
• Lead and drive the care team to ensure high-quality care.
• Support a culture of compassionate, family-focused hospice care.
